We desire that you will have a positive experience from Dr. Lloyd’s practice. The following guidelines have been established to help improve the productivity of office visits and the communication between you and our staff.
Neurosurgery is a specialized surgical field of medicine. Most of Dr. Lloyd’s time is spent at the hospitals with patients who are having surgery or have recently had surgery. Dr. Lloyd sees patients for appointments in our clinic on Monday and Thursday from 11:00 a.m. until 5:00 p.m.
Our office is staffed as follows: Monday and Thursday, 9:00 a.m. to 5:30 p.m., Tuesday, 9:00 a.m. to 2:00 p.m., and Friday 9:00 a.m. to 12:00 p.m. There is no reception staff in the office on Wednesday. Please understand that due to Dr. Lloyd’s surgery schedule, he may not be able to return your call the same day but will make every effort to call you as soon as possible after he receives your message,
In order to obtain proper clinical evaluation by Dr. Lloyd, it is imperative that you bring along x-ray films or CDs and the printed narrative results of any diagnostic tests you have already had done. This includes plain x-rays, MRI scans, CT scans, special diagnostic studies, etc. DO NOT MAIL these to our office.
Please prepare questions about your symptoms, diagnosis and treatment options in advance. Results of diagnostic tests ordered by Dr. Lloyd will be reviewed with you at your return visit. If your employer requires a work release form or other written documentation, it is your responsibility to request Dr. Lloyd’s signature at your office visit and submit it promptly to your employer. Dr. Lloyd will not “back date” forms.
Disability forms can be dropped off and picked up on Tuesdays and Fridays. There is a fee of $15.00 per form. Please allow 3 business days for completion of forms.
Please plan ahead for your medication needs, and allow at least 3 business days advance notice for prescription refill requests.
